1st Case-Feeling pain and tender at right hypocondriac region

ok...this patient complained he's having pain(memulas)at his right hypochondriac region..admitted this morning and his body temperature spiked reaching 40 degree Celsius...Murphy Sign is +ve...Someone please help me to make differential diagnosis...TQ

p/s-not to forget..before he is admitted, he claimed to be at the jungle(hunting maybe)..and he's and Orang Asli..

  1. by now-the doctors is going to rule out Ascending Cholangitis..thanks Franco for your reply on FB..
    u suggested malaria, leptospirosis, cholelithiasis, choledocolithiasis, hepatitis..i add some more..injured(blunt object)
