Body Region


1st Case-Feeling pain and tender at right hypocondriac region

ok...this patient complained he's having pain(memulas)at his right hypochondriac region..admitted this morning and his body temperature spiked reaching 40 degree Celsius...Murphy Sign is +ve...Someone please help me to make differential diagnosis...TQ

p/s-not to forget..before he is admitted, he claimed to be at the jungle(hunting maybe)..and he's and Orang Asli..

Eczema

Eczema (or dermatitis) refers to a group of chronic skin disorders that primarily involve the epidermis and include atopic, contact, stasis, seborrheic, nummular, dyshidrotic, generalized, or localized scratch dermatitis. Type depends on cause and location on body, but treatment is generally the same. Family history of allergic rhinitis, asthma, or atopic dermatitis often exists. Neurodermatitis is used to describe eczematoid rashes that seem to have a major stress-related component.

Management for Gastric

in case of emergency,
1)allow rest
2)iv slow bolus Ranitidine 2ml stat/ or Intramuscular
-IM Buscopan(Hyoscine Butylbromate)

patient can be discharge with
-Tab. Ranitidine 150mg OD x 1/12
-Syr. Mist Magnesium Trisilicate(MMT)10ml
-Tab. Gelusil 2/2 TDS x 5/7

in case patient looking pale, check for Malaena Stool, to see if pt had UGIB/LGIB..if any, inform your medical officer for further management.
